√ Items below marked with a CHECK MARK are necessary for a fully completed application
∗ Provisional Acceptance Needs – Items below marked with a STAR ICON (∗) are necessary for provisional acceptance. Submission of these items will allow your application to be reviewed and moved forward for provisional acceptance. Once provisionally accepted, you can begin the program. Then, you will have more time to submit the remaining documents (typically by the end of the 1st term) as specified in your provisional acceptance letter from the college.

√ Official transcripts from all undergraduate and graduate degrees, certifications and courses must be submitted
∗ Master’s Transcript with 3.0 GPA – An unofficial copy of your master’s transcript will suffice for provisional enrollment
3 Professional Documents
√ Personal statement, Resume & Certification
∗ Resume – Showing 3 years of experience under your license OR at least 1 year with the understanding and agreement from the applicant that you will complete 3 years before the end of the program or you will be ineligible for SBL/SDL certification per state requirements

4 ProfessionalReferences & InternshipCommitment FormSign Off
√ Two letters of reference and one signature on the Internship Commitment Form – these documents must total three separate individuals and one of the three must be a current supervisor.
